Follow Steps 1 & 2 to register & pay on-line:
Step 1: Enter your Name, Email, Subject & Requested Class with phone number. Click
on SEND.
Step 2: Click the SQUARE © LINK to pay $90 for class and required ebook. Payment will
appear on your statement as SQ*CHANCE FOR LIFE CPR. Upon successful payment,
you will be registered.
Cancellation Policy:
We require 48 hours notice prior to day of class to receive credit for a future class. The future class must be completed in 30 days.
